| This is to certify that ____________________________ has purchased the following parrotlet from B/C Aviculture on the ______ day of ________________, ________: Band number ___________________ Sex ___________________ Hatch Date ___________________ B/C Aviculture guarantees the parrotlet(s) is healthy and free from disease at the time of purchase. Should the bird become ill or die from disease within three (3) days of purchase and is determined by a certified avian veterinarian that the parrotlet was sick upon arrival, the buyer shall have the option of a refund or a replacement parrotlet of equal value. B/C Aviculture will be notified immediately should the parrotlet become ill or this guarantee will be invalidated. B/C Aviculture will reimburse buyer for all prescribed medications but B/C Aviculture reserves the right of taking the parrotlet and having it treated by its own veterinarian. Should buyer request another Parrotlet, B/C Aviculture will pay for all expenses in replacing the Parrotlet including shipping. The bird(s) is a hand-fed baby parrotlet and many hours have been expended in its care, development and taming to ensure a happy, healthy and well-adjusted pet.
